I-134 was the most confusing form for me too.
I let my lawyer fill it for me, and she did tick the "intend to" but did not fill the area in question.

Hi Attorneys,
Recently I have filed my H1B petition . However due to some emergency I need to quit my current organisation (Company A) who has sponsored the H1B . My questions are :
1. Would there be any adverse effect if in future Company B files H1B for me ?
2. What generally happens if I file H1B and am not able to attend the interview ?
3. Is there any way Company A can revoke the H1B petition after I resign before H1B interview ?
Would appreciate a quick a reply .

Let us be very clear, we are MORE THAN WILLING to associate with any org that can be beneficial to us. One of the members of IV had established contact with USINPAC and it has been a very fruitful association
If anyone can do the same thing with any other org (could be 'Chinese Physicians Association', or 'Russians in America' or 'Canadians in USA' or even 'Americans for High Skilled Immigrants' - please note that I made up all the names, but please get the gist) please take the inititative and let us know how it goes.
Also note, that even if we associate with an org primarily working for a particular country, all our issues will benefit EVERYONE from every country. It is impossible to press for an amendment like 'Ability to file I485 without regard to visa number for Indian Nationals only'. The US Government doesn't allow that and neither will IV go in that direction.
The keywords here are 'publicity and exposure'. We really need that to protect our amendments in the Senate and the House Conference

I Just heard back from my Fortune 500 company that they will accept the replacement card receipt notice...so it may work. It will help to specifically ask the HR/Lawyer to check the Receipt rule.

the 140 was denied because of missing job description from my first employer, it took 5 months for the MTR to be processed.

My employment based I-485 application has been pending since Aug'2004. My labor priority date is May 2003. I came to know from immigration officer from local office few weeks ago that name check process was cleared very recently. But, USCIS has responded to a recent service request as follows:
The processing of your case has been delayed. We are currently awaiting the results of required security checks on this case. These background checks are required of all who apply for the immigration benefit your are seeking. We will make every effort to make a decision on this case as soon as security checks are complete. If you do not receive a decision or other notice of action from us within 6 months of this letter, please contact us by calling our customer service number provided down below.
If name check is cleared what else is pending? Is information provided by the immigration officer at local USCIS is wrong regarding the security checks.
Could you please throw some light on these background checks. What are my options. How many more years do i have to wait? Your valuable information in this regard is greatly appreciated.

yes..I was sent inside for 2nd checking (which is common for AP)
infact officer sympathized for AOS delays..so it depends.
